Choose the report’s purpose

Decide whether the client needs confirmation of hours, completion of agreed activities or an account of material service changes. A clear purpose helps keep the report concise. Do not ask for broad observations about unrelated people or private conversations merely because a professional was present during the visit. A finance-focused summary may require different detail from a service-quality review, so decide whether separate concise records would be clearer.

Use relevant factual entries

Record the agreed start and finish, approved extensions and practical exceptions that affect the booking. Distinguish confirmed facts from items needing clarification. Avoid speculation about the visitor’s personal relationships, commercial decisions or health; those matters do not ordinarily help reconcile an accompaniment assignment or assess its delivery. If an entry is disputed or incomplete, mark it for clarification rather than turning an uncertain recollection into a definite statement.

Limit the recipient list

Identify the client contact authorised to receive the report and whether finance needs a separate summary. The full document should not automatically go to every person who helped arrange the visit. Keep commercial evidence and sensitive operational information separated where that makes the record more proportionate and easier to manage. Ask recipients to use the agreed handling process instead of forwarding the document into unrelated group conversations about the visit.

Follow agreed retention arrangements

Ask how the record will be stored and retained under the applicable agreed process. Do not create additional copies without a purpose. If the client needs a correction, send a precise request through the authorised contact so the final record accurately reflects the service rather than leaving competing versions in circulation. The booking reference and report date can help distinguish the corrected version without reproducing unnecessary personal information in the filename.
